The Office of the Child Advocate Fatality Review Panel Reports and Recommendations

The Office of the Child Advocate’s Fatality Review Panel publishes periodic reports examining child fatalities and near fatalities involving children connected to the Department of Children, Youth & Families. These reports include recommendations aimed at strengthening system response and improving outcomes for children and families.
